Baptismal Verification Statement

Baptismal records reflect a sacred religious act performed in the name of the Father, the Son, and the Holy Spirit, in accordance with Christian faith and practice. Verification confirms that the baptism was conducted by an authorized minister and properly recorded by the church.

Ministerial Verification Statement

Ministerial records verify that an individual has been ordained or commissioned by the church to perform religious duties. These records confirm standing at the time of issuance and do not constitute civil licensure.
